Link with your networked practices

Through our app, Clusters and Primary Care Networks can develop their own page and communicate with their Cluster / PCN population.

When a patient registers their surgery within the app, the surgeries app dashboards also has a link to the Clusters’ / PCN’s section of the app giving networked practices a direct link to their patients
population.

Clusters / PNS’s can:

• Send push notifications to all cluster / PCN patients
• Explain how Clusters / PCN’s work, what they do and what they work to achieve
• Communicate current projects through the Community Projects page
• Communicate Social Prescribing Projects
• Contribute to the ‘What’s On’ calendar in the main App
• Link to Cluster / PCN podcasts
• Contribute to local news
