If ear wax is not causing you any problems, it is best left alone and you do not need to continue with this referral form. The ear is self-cleaning and often the wax will come out without you needing to do anything.

Sometimes ear wax can build up enough to cause hearing loss and other problems.

The service provides assessment, treatment and if required onward referral to Pelvic Health Physiotherapy, Continence, Pain Management & Gynaecology within and outside Powys. Falls Assessment referral form

Please complete the following form to request assessment of any individual over 65 years of age who has suffered a fall or is fearful of falling, and has consented to this referral.

Referrals will be reviewed by a Multidisciplinary Team of Healthcare Professionals and the person who has been referred will be contacted to arrange an assessment within 2 weeks.

Help Me Quit | Stop Smoking Services In Wales

Help Me Quit is the single point of contact for smokers who want to stop smoking in Wales.

All Help Me Quit services in Wales will provide you with structured support on preparing to quit, quitting, staying stopped and your smoke-free future. Sessions are always delivered by trained stop smoking experts. There are a number of services in Wales. All services are free and will give you the best chance of quitting smoking for good. When you choose a Help Me Quit service, your stop smoking expert will discuss the different types of stop smoking medications which are available to help you quit

Hearing aid clinics

The NHS provides free hearing aids to patients with impaired hearing. Your GP can refer you to the nearest clinic for a hearing test. Many clinics offer repairs and batteries.

Integrated Autism Service

The services provide adult autism diagnostic assessment  (sometimes jointly with other services), support and advice for autistic adults, parents/ carers, and professionals.

SilverCloud - Free Online Mental Health Therapy

People across Wales can now access free online therapy without needing to go through their GP.

People aged 16 and over experiencing mild to moderate anxiety, depression or stress can sign-up for a 12-week course of SilverCloud online therapy via their smartphone, tablet, laptop or desktop computer.

The roll-out of direct access to online therapy for Wales’ entire 16+ population recognises that people need immediate help in managing their mental health and wellbeing as the impact of COVID-19 continues to be felt, and reduces barriers to accessing this support.

There are a wide range of activities both gym based and class based to choose from for patients that have been through rehabilitation programmes. You will be able to access a wide range of opportunities and these will be available between 4 and 48 weeks of the programme (depending on medical condition).
